The short answer
Arizona residents can self-exclude from gambling through Arizona Department of Gaming Self-Exclusion Program, run by Arizona Department of Gaming (ADG), Division of Problem Gambling. Exclusion periods: 1 year, 5 years or 10 years. Two separate exclusion lists: one banning you from all Arizona tribal casino gaming facilities, and one banning you from event wagering (sports betting, retail and mobile apps) and paid fantasy sports contests; you can enroll in either or both.

Self-exclusion alone isn't enough
Arizona's program only binds operators licensed in the state — offshore sites and crypto casinos ignore it. The setup that holds is layered: self-exclusion plus a gambling blocker on your devices plus a gambling block on your bank card.
