How to enroll

1. 1 In person by appointment at the Arizona Department of Gaming, which walks you through the form, notarizes it, and takes your photo same-day (email selfexclusion@azgaming.gov or call 602-771-4263)
2. 2 By mail or email: download the self-exclusion form from gaming.az.gov/exclude, have it notarized, attach a color head-and-shoulders photo, and submit it

What to watch out for

Casino and event-wagering/fantasy exclusions are separate lists and must each be requested. Exclusions cannot be revoked before the chosen term ends. The state lottery is not covered. The form requires notarization and a photo. Helpline: call 1-800-NEXT-STEP or text NEXTSTEP to 53342.

Self-exclusion alone isn't enough

Arizona's program only binds operators licensed in the state — offshore sites and crypto casinos ignore it.
