What to watch out for
One of the broadest programs in the US - a single form covers nearly every legal gambling type in the state. After a 1-, 3-, or 5-year term expires the person is automatically removed unless they renew within 30 days; lifetime exclusion is permanent unless formally revoked. Tribal casinos maintain their own exclusion processes.
